To Apply for Assistance from DVWFL Funds, the Candidate must::

• Have been diagnosed with some form of cancer
• Submit proof of having received treatment from an Oncologist, Hospital or Cancer Treatment Center
• Submit an application to be approved by the Executive Board of Death Valley Walk for Life, Inc.
